Build Stronger Bones Through Better Nutrition
Osteoporosis causes bones to become weaker and more likely to fracture. Because the vertebrae are particularly vulnerable, osteoporosis can lead to compression fractures, back pain, loss of height, and changes in posture.
Although nutrition cannot replace osteoporosis medication when treatment is necessary, eating well provides your body with the building blocks it needs to preserve bone strength and support your treatment plan.
The Most Important Nutrients for Bone Health
Calcium
Calcium gives bones much of their strength and structure. When your diet does not provide enough calcium, your body may take calcium from your bones to maintain other essential functions.
Daily calcium goals from food and supplements combined are generally:
Women age 50 and younger: 1,000 mg
Women age 51 and older: 1,200 mg
Men age 70 and younger: 1,000 mg
Men age 71 and older: 1,200 mg
Try to meet your calcium needs through food first. A supplement may be appropriate when your diet does not provide enough.
Good sources of calcium include:
Milk, yogurt, cottage cheese, and cheese
Calcium-fortified soy, almond, or oat milk
Sardines or canned salmon with bones
Calcium-set tofu
Collard greens, kale, turnip greens, and broccoli
Fortified cereal and orange juice
Almonds and beans
Check the Nutrition Facts label. A food containing 20% of the Daily Value for calcium provides approximately 260 mg under current U.S. labeling standards.
Vitamin D
Vitamin D helps your digestive system absorb calcium and supports normal bone mineralization and muscle function.
Few foods naturally contain large amounts of vitamin D. Helpful sources include:
Salmon, trout, tuna, and other fatty fish
Egg yolks
Vitamin D-fortified milk and plant-based milk
Fortified cereals
UV-exposed mushrooms
Some patients need a vitamin D supplement, particularly when a blood test shows deficiency. Do not take very high doses unless they have been recommended and monitored by a healthcare professional. Excess vitamin D can cause dangerously high calcium levels and kidney complications.
Protein
Bone is living tissue, and protein is necessary for maintaining its underlying structure. Protein also helps preserve muscle strength, which supports balance and may reduce the risk of falls.
Include a source of protein with each meal:
Fish, chicken, turkey, eggs, or lean meat
Greek yogurt, cottage cheese, or milk
Beans, lentils, peas, or soy
Nuts, seeds, or nut butter
People with kidney disease or other medical conditions should speak with their healthcare provider before significantly increasing protein intake.
Magnesium, Vitamin K, and Other Nutrients
Magnesium, vitamin K, phosphorus, zinc, and vitamin C also contribute to bone formation and maintenance. Most people can obtain these nutrients by eating a varied diet that includes:
Green vegetables
Beans and lentils
Nuts and seeds
Whole grains
Berries and citrus fruit
Fish and lean protein
A balanced diet is generally more helpful than relying on a large collection of individual supplements.
A Simple Bone-Healthy Plate
For most meals, aim to build your plate with:
One-half vegetables and fruit
One-quarter protein
One-quarter whole grains or another high-fiber carbohydrate
One calcium-rich food or beverage
Example Day
Breakfast
Greek yogurt with berries, walnuts, and a calcium-fortified cereal
Lunch
Grilled chicken or tofu with mixed greens, beans, vegetables, and an olive-oil-based dressing
Snack
Cottage cheese, a fortified plant-based beverage, or an apple with almond butter
Dinner
Salmon with broccoli and brown rice or quinoa
Habits That Can Weaken Bones
Nutrition is not only about what you add. Certain habits may work against bone health.
Avoid smoking and nicotine
Smoking is associated with poorer bone health and may interfere with healing after a fracture or spinal procedure.
Limit excessive alcohol
Heavy alcohol consumption can weaken bones, impair balance, and increase the risk of falls. Ask your healthcare provider whether alcohol is safe with your medications.
Avoid excessive dieting
Being underweight or losing weight too rapidly can contribute to bone and muscle loss. Patients trying to lose weight should continue consuming adequate protein, calcium, vitamin D, and overall calories.
Be careful with excess sodium
Diets high in processed and packaged foods often contain large amounts of sodium. Choose fresh foods more often and check food labels when possible.
Should You Take a Calcium Supplement?
More calcium is not always better. First estimate how much calcium you regularly receive from food. A supplement should generally provide only the amount needed to make up the difference between your diet and your daily goal.
Calcium supplements can cause constipation and may interact with thyroid medication, certain antibiotics, iron, and other medicines. They may not be appropriate for some people with kidney stones, kidney disease, or abnormal calcium levels.
Discuss calcium supplementation with your primary care physician, osteoporosis specialist, pharmacist, or registered dietitian.
Nutrition Is Only One Part of Osteoporosis Care
Strong bones also depend upon:
Weight-bearing physical activity
Resistance or strength training
Balance and fall-prevention exercises
Avoiding tobacco
Appropriate testing and medical treatment
Taking prescribed osteoporosis medication correctly
Patients with osteoporosis or a previous vertebral compression fracture should receive individualized exercise guidance. Certain bending, twisting, lifting, or high-impact movements may not be appropriate.
When to Seek Medical Attention
Contact a healthcare professional if you experience:
Sudden or severe back pain after a minor movement or fall
New loss of height
Increasingly rounded posture
Difficulty standing or walking because of pain
A known compression fracture
Persistent pain despite rest and conservative care
Sudden back pain in a person with osteoporosis may represent a vertebral compression fracture and should be evaluated.
